Chronic systemic inflammation refers to a prolonged, low-grade immune response where the body’s inflammatory chemicals continuously circulate and damage the healthy tissues.
In Indians, this is a major, hidden driver of the country’s high rates of metabolic diseases like type 2 diabetes, cardiovascular issues, and obesity.
While a single nationwide, population-wide prevalence statistic is difficult to pin down, clinical research indicates that chronic systemic (low-grade) inflammation is highly prevalent in India. Cardiometabolic diseases including type 2 diabetes mellitus, cardiovascular disease (CVD), and metabolic syndrome are rising sharply among South Asians, both within the countries of origin and in diaspora communities worldwide.
This surge is not only due to conventional risk factors but also driven by chronic systemic inflammation that accelerates disease onset and progression.
South Asians develop cardiometabolic disease at much earlier ages and at much lower body mass index (BMI) than other ethnic groups, in part due to their greater tendency toward visceral fat accumulation, which is metabolically active and proinflammatory.

In a population-based study in Chennai, India, age-adjusted diabetes prevalence among Asian Indian adults was over 28%, nearly 3 times that of White adults in the United States, despite lower waist circumference and BMI.

Elevated lipoprotein(a) [Lp(a)], a genetically determined particle with pro-atherogenic and pro-inflammatory properties, is significantly more common in South Asians. Levels above 50 mg/dL for Lp(a) are associated with a 2-fold higher risk of CVD, seen in nearly 25% of this population.
These inflammatory and genetic vulnerabilities contribute to the earlier and more aggressive course of cardiometabolic disease in this population as we are witnessing in our daily lives.
