What Current Clinical Evidence Reveals About Nutrient Adequacy, Bone Health and Long-Term Health Risks

Introduction

Vegan diets have gained worldwide popularity over the past decade for ethical, environmental and probable, potential health reasons. Millions of people have eliminated all animal-derived foods—including meat, fish, eggs and dairy—from their diets.

Research consistently shows that vegan diets need very careful planning as these are associated with lower saturated fat intake and higher fibre consumption with missing out on certain key, essential and critical Vitamins and minerals. The Academy of Nutrition and Dietetics (AND) states that, in adults, appropriately planned vegetarian and vegan dietary patterns can hopefully, be nutritionally adequate but may require regular supplements to overcome some structural deficiencies from vegan foods.

The same position paper also heavily emphasizes that careful planning is essential to reduce concerns about nutrient inadequacies. This distinction is critical. Nutritional adequacy is not automatic; it depends on thoughtful food selection, fortification and, in many cases, supplementation.

For clinicians, dietitians and consumers, the important question is therefore not whether vegan diets are “good” or “bad,” but whether they consistently provide all nutrients required for optimal long-term health.


Evolution of the Evidence

The 2025 Academy Position Paper differs from its earlier guidance by focusing specifically on non-pregnant, non-lactating adults. It does not evaluate children, pregnancy or lactation because those populations require full and extensive foods to meet nutritional requirements. Pregnants, lactating women and children / adolescents are at a greater risk of critical nutritional deficiencies from vegan diets.

This narrower scope reflects the complexity of ensuring nutritional adequacy during periods of rapid growth and increased physiological demand, and potentially may serve as an evidence against vegan diets themselves.

Nutrient Challenges in Vegan Diets

The greatest nutritional concern with vegan diets is not calories—it is micronutrient density and bioavailability.

1. Vitamin B12: The Most Critical Nutrient – Vitamin B12 is naturally present almost exclusively in foods of animal origin. Without fortified foods or supplements, long-term vegans are at high risk of deficiency. Vitamin B12 deficiency may cause:

  • Megaloblastic anaemia

  • Peripheral neuropathy

  • Cognitive decline

  • Elevated homocysteine

  • Irreversible neurological damage if untreated

Professional nutrition organizations therefore recommend reliable B12 supplementation for people following vegan diets.

2. Iron: Quantity Does Not Equal Bioavailability – Plant foods may contain substantial amounts of iron, but this is non-heme iron, which is generally absorbed less efficiently than heme iron from meat, poultry and fish.

3. Zinc – Zinc is essential for: Immune function, Growth, Wound healing, Male Fertility, Reproduction and many Enzyme activities. Although legumes and whole grains contain zinc, phytates reduce its absorption. Consequently, vegans may require higher dietary zinc intake to achieve equivalent physiological availability.

4. DHA and EPA Omega-3 Fatty Acids – Alpha-linolenic acid (ALA) from flaxseed, chia and walnuts can be converted into DHA and EPA, but human conversion efficiency is very limited, if any. DHA is a structural component of:

  • Brain tissue

  • Retina

  • Neuronal membranes

Many clinicians therefore recommend algal DHA supplements for individuals consuming no marine foods.

5. Calcium and Vitamin D – Eliminating dairy removes one of the richest dietary sources of calcium. Unless fortified alternatives are consumed regularly, calcium intake may become suboptimal. Vitamin D deficiency remains common globally and often requires supplementation regardless of dietary pattern.

6. Iodine – Milk and seafood contribute substantially to iodine intake in many populations. Vegans relying on non-iodized salt may be vulnerable to iodine deficiency, increasing the risk of thyroid dysfunction.

7. Choline – Choline is essential for: Cell membrane integrity, Liver function, Neurotransmitter synthesis and Brain development. Eggs are among the richest dietary sources. Many vegan diets provide lower choline intake unless carefully planned.

8. CoQ 10 –Coenzyme Q10 (CoQ10) is an essential component of the mitochondrial electron transport chain and an antioxidant. While Vegans may get some CoQ10 from plant sources – a large study from Japan demonstrated that – Plasma concentrations of total CoQ10 were found to be significantly lower in the vegetarian/vegan group compared with the omnivore group.


Bone Health: What Does the Evidence Show?

One of the most debated questions concerns fracture risk. Several observational studies have reported lower bone mineral density among vegans.

More importantly, recent systematic reviews have suggested an increased risk of hip fractures. One recent meta-analysis reported that vegans experienced an approximately 75% higher relative risk of hip fracture compared with meat eaters. However, the authors also rated the certainty of this evidence as very low, meaning the estimate should be interpreted cautiously and not as definitive proof of causation.  Possible explanations include:

  • Lower calcium intake

  • Lower protein intake

  • Lower BMI

  • Reduced vitamin D

  • Lower vitamin B12

  • Lower intake of vitamin K2

Importantly, fracture risk is influenced by many factors beyond diet alone.


Protein Quality Matters

Total protein intake is only part of the equation. Protein quality depends upon:

  • Essential amino acid composition

  • Digestibility

  • Bioavailability

Animal proteins generally achieve higher Digestible Indispensable Amino Acid Scores (DIAAS) than most plant proteins. Combining legumes, soy foods, whole grains and nuts can improve amino acid balance, but this requires dietary planning and also extensive risks from anti-nutritional factors in plant foods. Soy foods are considered estrogenic.

Older adults may be particularly vulnerable because anabolic resistance increases protein requirements with ageing.


Implications for Older Adults

Maintaining muscle mass becomes increasingly difficult after the age of 60. Older adults require:

  • Higher protein quality

  • Adequate leucine

  • Vitamin B12

  • Vitamin D

  • Calcium

Failure to meet these requirements increases the risk of: Sarcopenia, Falls and Fractures and Functional decline. All NOT Worth taking risk with a Vegan Diet. For this reason, nutritional assessment becomes particularly important in elderly vegans.


Indian Perspective

India presents unique nutritional challenges. Large sections of the population already consume predominantly vegetarian diets, with relatively lesser usage of dairy products, a semi-vegan diets. Its no wonder India has one of the highest:

  • Disease burden of Type 2 Diabetes

  • Extensive, pervasive, persistent Vit B12 deficiencies

  • Extremely high levels of Iron Deficiency Ananemia by excessive consumption of non-heme iron sources

  • Zinc deficiencies especially in young age

Transitioning from an already, virtual semi-vegan, vegetarian diet to a completely vegan diet may further reduce intake of several nutrients if dairy products are also removed without any appropriate nutritional replacement in form of supplements.

This makes dietary counselling especially important. We best none of our Gran-parents were began or even were aware of the concpt of veganism.


Practical Take-Home Messages

Individuals choosing vegan diets should:

  • Supplement Vitamin B12 consistently and consciously with a good quality, more bio-available supplement, including injectable supplements

  • Monitor vitamin D status carefully and if considering a supplement, only good, more bioavailable supplements are “animal origin” Vit D3 only

  • Ensure adequate calcium intake – and how, honestly even we are at a loss

  • Include high-quality protein sources such as soy foods, legumes and pulses which have high carbohydrate contents as well as have limiting amino acids

  • Consider algal DHA supplementation – not sure if it fits with vegan philosophy

  • Consume iodized salt or other reliable iodine sources

  • Periodically assess nutritional status with healthcare professionals – increased cost, time and efforts


Conclusion

Modern nutrition science no longer frames the debate as “vegan versus non-vegan.” Instead, the evidence indicates that diet quality matters more than dietary labels. Its your body, you got only 1 chance, so make all conscious and cautious choices for your body with your diet and lifestyle. 

A vegan diet can support good health only when it is carefully planned, nutritionally balanced and appropriately supplemented – but all of this is oxymoron since balancing requires usage of animal meats and dairy food sources.

Conversely, poorly planned vegan diets may increase the risk of clinically important nutrient deficiencies, particularly vitamin B12, calcium, iodine, zinc, DHA and high-quality protein.

For clinicians and consumers alike, the most evidence-based approach is not ideological—it is nutritional adequacy.
